Your patients fill out forms every visit. Most of those forms are useless. Here is how to build ones that actually help.

Every gynecology office has intake forms. Name, date of birth, reason for visit, last menstrual period. Maybe a checkbox for “pelvic pain” and another for “heavy periods.” The patient checks a box, the medical assistant glances at it, and the visit begins.

That form does almost nothing to help the clinician identify endometriosis, PCOS, PMDD, adenomyosis, or any of the conditions that present with overlapping pelvic symptoms. It does not ask about pain severity. It does not ask about dyschezia. It does not capture the pattern of symptoms across the menstrual cycle that distinguishes one condition from another.

It is not the clinician’s fault. Building a comprehensive, evidence-based screening questionnaire takes time, design skill, and clinical knowledge. Most clinicians have the clinical knowledge. They do not have the other two.
